Your amazon.com order cannot be shipped - amazon account locked

I tried to place order on both amazon.com and amazon.uk using my USA card. I got email to send statement, which i did, they replied saying not enough info and wanted me to reupload however i was no more able to log into my amazon to upload it again. After i emailed them, they said i need to get my bank to fax it to them. Help please.

Can you please help me? I stumbled upon this website through googling. My account was locked because i tried to shipped my order in my home country philippines. They want me to provide all the receipts of the Gift Card that I've used..
